A descriptive prospective study of 297 patients over 65 that receive attention at the family physicians' offices no 19 and 21, corresponding to "La Rampa" Polyclinic, Plaza de la Revolución, Havana City, was conducted from January 1st trough December 31st, 2003, aimed at identifying the patients with cognitive deterioration and at knowing some of their clinicoepidemiological characteristics. A survey was done with the variables of interest. Folstein's Minimental State was used as a tool to measure cognitive deterioration. The severity degree of the cognitive deficit and the functional disability of the affected patients were also evaluated. To analyze the data, percentages were calculated and the results were presented in tables and in a figure. 41 patients (13.8 %) showed cognitive deterioration. Most of them (24.4 %) corresponded to the age group 85-89. Females were the most affected (68.3 %), whereas 56.1 % of the patients had a low educational level. Arterial hypertension (51.2 %), ischemic heart disease (34.1 %), and smoking (24.4 %) were the most frequent risk factors in the sick with cognitive deficit. Cognitive deterioration was mild in 27 patients (65.8 %), moderate in 8 (19.6 %) and severe in 6 (14.6 %). 12.1 % of those presenting with cognitive deterioration had a severe functional disability, 9.7 % moderate, and 34.4 % mild. The most affected cognitive function was the mediate memory, followed by attention-calculation and language.
Se realizó un estudio descriptivo prospectivo en 297 pacientes mayores de 65 años de los Consultorios No. 19 y 21, pertenecientes al Policlínico “La Rampa”, Plaza de la Revolución, Ciudad de La Habana, en el período comprendido entre el 1º de enero y el 31 de diciembre del año 2003, con el objetivo de identificar los pacientes con deterioro cognitivo y conocer algunas de sus características clínico epidemiológicas. Se aplicó una encuesta con las variables de interés. Se utilizó como instrumento de medición del deterioro cognitivo el Minimental State de Folstein y se evaluaron además, el grado de severidad del déficit cognitivo y el grado de incapacidad funcional de los pacientes afectados. Para el análisis de los datos se efectuó el cálculo de porcentajes y los resultados se presentaron en tablas y una figura. Presentaron deterioro cognitivo 41 pacientes (13,8 %), correspondiendo la mayoría de ellos (24,4%) al grupo de edad de 85-89 años. El sexo femenino fue el más afectado (68,3 %) y el 56,1 % de los pacientes mostraron un bajo nivel escolar. La hipertensión arterial (51,2 %), la cardiopatía isquémica (34,1 %) y el hábito de fumar (24,4 %) fueron los factores de riesgo más frecuentes en los enfermos con déficit cognitivo. El deterioro cognitivo fue leve en 27 pacientes (65,8 %), moderado en 8 (19,6 %) y severo en 6 (14,6 %). El 12,1 % de los que presentaron deterioro cognitivo tuvieron incapacidad funcional severa, un 9,7 % moderada y el 34,4 % ligera. La función cognitiva más afectada fue la memoria mediata, seguida por la atención-cálculo y el lenguaje.
